Incontinent after a fall?

A couple of mornings ago I slipped and fell in our garage landing hard on my bottom. At the time of the fall I had been about to urinate and was rushing to get to the toilet. The very second my bottom hit, I began urinating. Automatically, I tried to stop the flow, but it wouldn't stop. I ended up wetting all over myself. After this my bottom and vaginal area had a numb but sore feeling. A short time later I began feeling a slight urge to urinate when suddenly I started wetting. Since the incident about 48 hours ago I've been wetting myself every few hours. I have tried to go to the toilet in between accidents but can't seem to use it. Then sometimes no more than fifteen minutes later end up wetting myself. My sister is ashamed of my situation and humiliated me last night by purchasing me a pack of adult diapers. I'm 32 with three early teenaged kids. Prior to the fall I had experienced some of what I think is called stress incontinence where I'd wet a little when I cough and sneeze. Could the fall have caused me to become totally incontinent? Is it possible I've did some type of damage to myself? I'm ashamed about the thought of going to a doctor, especially now that I'm having to diaper myself. It is truly humiliating now as it is. My family tries to act innocent, but I feel so awkward about knowing they look at me and know I have a diaper on.
