Has Molymyalgia rheumatica been ruled out or giant cell arthritis. Usually it is seen in elder people, but have seen in young forties. This disease causes the nerve endings to rise outside the joint space and cause pain. Easy blood test can tell. I assume juvenille arthritis has already been eliminated also.
