Has your doctor or phys therapist ruled out adhesive capsulitis? The shoulder capsule can shrink with disuse when you have an injury anywhere around that area. This happened to me and I could not lift my arm ... terrible pain radiated into my neck and chest. After a few months physical therapy I have no pain and full range of motion, wish I'd treated it sooner. I hope this helps, good luck to you.
