You can easily find out. Go to the radiology or wherever they have the records of your surgery and every hospital has an office like this. Tell them you want a copy of your procedure and give them the date of your surgery and doctor. They will make you a copy and all you do is sign for it. You have to go in person. People do this all the time. I go after any procedure I have had done and keep my own file as I forget over the years exactly what has been done. You are always needing this info. when you go to a hospital or a new doctor any way, so it is just good practice.
