You may think this sounds silly, but you might want to get an x-ray of your head. I had the same problem (pain in shoulder and neck) and it was finally discovered after an x-ray and bone scan that the pain was referred and was coming from my the base of my skull. It is called fibrous dysplasia and is a bone disorder which causes abnormal development of fibrous tissue within the bones. It just takes finding the right doctor to figure it out.
