Maybe you are not allergic to lettuce, but you might be allergic to a preservative used by some restaurants to preserve the lettuce. So, if you can find out if they used a preservative, you might be able to find out what you are allergic to. Also, you might try to find a place that does allergy testing, and they might be able to pinpoint your allergy.
