I am not a physician and hesitate to give medical advice. However, as you say you have seen doctors and they have ruled out lupus which I also felt was the cause. On a long shot since nothing seems to help, here are my suggestions. First, STOP eating meat and dairy products, try it for 3 months and see how you feel. Go to your local health food store and buy Co-Enzyme Q10 which is commonly called CO Q10 (co-Q-10) it is normally found in your body but when levels drop, health problems develop. All C0-Q10 are made in Japan where it was discovered and sent to the U.S. where it is sold under many brands. Buy the ones containing at least 50 milligrams or 60 milligrams. Dosage is on bottle label, it is very safe. A man bought some for his mom who had a very bad heart and doctors gave up on her, she could hardly walk 25 feet. He accidently gave her a much higher dose and to everyones's surprise, she was soon walking a mile a day. The second is to buy some Omega-3 Fish oil capsules. Fish oil seems to counter many ailments so try it. You may also try a tablespoon of Olive oil daily washed down with fruit juices. I don't hesitate recommending these to you as they are perfectly safe and could help. Good Luck. p.s. eating salmon often is helpful as salmon contains Omega-3 oils.
